My understanding is that the 'tunica' limits corpora cavernosa size. This from Wikipedia:
"Blood can leave the erectile tissue only through a drainage system of veins around the outside wall of the corpus cavernosum. The expanding spongy tissue presses against a surrounding dense tissue (tunica albuginea) constricting these veins, preventing blood from leaving. The penis becomes rigid as a result."
The italian doctor who used vein grafts would cut the tunica to 'fix' this problem. Patients later became impotent, and the procedure was discontinued.
Some years ago I was in contact with an American plastic surgeon whose technique involved cutting the tunica, then slowly 'growing' the resultant scar tissue by pumping for 6 months post-op. One day his website suddenly went down and never came back up.
Perhaps some someone could asks Runels how his procedure overcomes the tunica limitation.

I thought I'd do this topic a favor and get some answers for all of us, something a bit more comprehensive.
I have spoken to the doctor again and he seems interested in an interview format thread. And for those who are wondering, its perfectly within the forum rules to conduct this interview (with the doctor directly) pending my approval (and I approve).
While I am considering this option for other doctors in the coming weeks/months, I thought I'd start with this one first because of how new this method is and how much popularity and inquiry this thread is getting.
So here is how it's going to go (once I get a confirmation back from Dr. Runels):
I will issue a new thread where you can post any question you have for Dr. Runels. The best 15-20 questions will be collected and sent to him via email. Questions that will receive the most consideration are one's that have NOT been answered by his website or by this current thread, and ones that require objective, clear answers. He will respond to the questions and email them back to be posted as a resource for the community.
I hope to have this done within the next week or two.
Any questions, concerns, etc please refer them to this thread, thanks.

I usually just read, not post. However, I am contemplating having this injection in December or January. On his video explaining the procedure did he say he injected himself twice? I believe I heard him say "I injected myself.......... and the next day I injected myself again (?). Did anyone else hear this? I'm going to listen to it again to make sure I heard him right. The volume on my Mac sucks with this video!

I just listened to the audio again and he does say that he injected his penis twice. The first time in the base and then 2 days later he injected his penis again this time more distally. Is this common practice for everyone or was it more of an experiment on himself? Is it one injection and then 2 days later another? He says he gained almost one and a half inches when fully erect.

He said he injected himself a few times in different places and the way he did is going to be patented!
I am concerned that if the injections are not done properly, there may be areas of the penis that grow more than others hence one may get an uneven looking penis.

OK, just got off the phone with Dr. Runels. He sounds like a genuine nice guy. He is in Alabama. He is not a Urologist, but he has been in many different fields of medicine and ER.
He told me he did this procedure on about 30 men. Only a few old men who had ED did not respond to this method.
He did it to him self and saw over an inch in girth and length. The length seems to respond to the treatment much later (what's new!) like in a few weeks to months, but the girth is almost immediate. A pump is used after the procedure to allow the PRFM to work properly.
He injects the penis shaft AND the glans and he claims it works well on both.
I asked him about the tunica limiting corpora cavernosa in this procedure and he said that the injection actually helped to increase the firmness of an erection.
He is not sure about repeat injections since this is still a new procedure but he suspects it may be less effective a second time.
He promised he will put me in touch with one of his patients to talk about his procedure.
We talked for a while, but I think this is the meat of the info I got. If one of his patients call me, I will let you know.
It may be wise to not have many people call him at once, he is not into the internet thing.
I strongly advised him to quickly apply for a patent for this procedure anyway, and he agreed...

I find it so hard to believe he could have gained anything like and inch in length and girth. I've done some pretty rough calculations, but they should be reasonably accurate and worked out the average alloderm procedure increases overall size of the penis by about 3.3 cubic inches (assuming a starting size of 6 by 5). I did this by saying the average girth increase is 0.75" and the shaft is a cylinder 5" long, by 5 inches round. I used 5"instead of 6", as the head/gland doesn't get enlarged with alloderm. Now if this injection increases a 6 by 5 penis to 7 by 6, then the overall volume increase is about 8.6 cubic inches. That is a huge increase. Unlike with alloderm this had the advantage of making the penis longer (real penis length, not lig cutting) and includes the head of gland getting bigger, which is a big reason the gains would be so much greater. But it just doesn't seem plausible that an injection of filler could do that. It really would be the holy grail of penis enlargement if it was true. If the results are exaggerated and in reallity only prove to be a third as effective, that agin is still bigger than what is produced by the average alloderm surgery. The girth gain wont be as great, but in terms of real size, ie overall volume, a 0.3" gain in length and girth, would be greater than a 0.75" gain with alloderm.

As i said in a previous post, there is a Dr in London, who I am seeing on the 29th who has done enlargement with Restylane, so i'll ask him about his injection technique. Restylane is hyaluronic acid...I don't know/heard of any single person happy about his hyaluronic acid injection...Most of you guys know that it tends to move in the penis because it's not attached, can leave lumps etc...if it was a success then probably everybody would be using that, cause doing the injection is relatively easy compared to the surgery

I don't want to ruin the mood here, but I would just urge everyone to exercise caution and ask themselves if they really want to be a guinea pig. Remember, there could be complications the doctor is not talking about, or doesn't know about because of the novelty of the procedure. Mustang, I know you had issues with the Elist implant, and I would hate to see you get hurt even more.
My wish is that this is a very successful method, but I would just tell everyone to be careful. The lack of professionalism in his website kinda scares me.
I don't have an agenda, and don't endorse any doctor. I just want you all to be safe.

Caution must be exercised and as much information found out before agreeing to receive this injection (or injections). One of the most basic things that Dr. Rundels can do is submit before and after pictures to see if this thing really works and speak with some of his patients. I really hope this technique works and if it entails so post operation work such as hanging for pumping, I wouldn't mind that. Good luck.

Hoddle: From what I understand, PRFM is not a filler. According to the info on Dr. Runels website, the solution that is injected into the corpus cavernosum reabsorbs after a couple of days. The size gains are due to the "growth factors" in the PRFM solution. I believe Runels is saying that the size gains are the result of actual new tissue and blood vessel growth. I believe this would also explain the length increase. I'm not sure if any kind of filler like Restylane could give a length increase. Anyway, that's the way I understood it. acu

Yes. Dr. Runels told me he uses one's blood to process and then obtain the PRFM and then injects it into the penis and glans where new cells grow and achieve increases in girth first then in length
