Hi donjuannabe, As I understand it, for virgin cases for lengthening procedures, Dr. Giunta cuts the suspensory ligament (I believe with a laser) then Dr. Giunta mandates mandatory stretching to prevent retraction. His one paragraph about using FFT with PRP for guys who lost length from an implant: "Dr. Giunta’s FFT technique using Platelet Rich Plasma can effectively reverse some of these losses, depending on the implant type and patient, and can add both length and girth to the patient who has penile implants." I suppose there might be some length gain by using the FFT with PRP, but this might be a question for idm and Dr. Giunta himself. You still might be required to stretch post operatively. Dr. Giunta recommends the Helping Hand Extender, which seems to be very similar to the autoextender. Hi Sheldon, in another post (think it was the bunk Elist thread) you used a quote from Dr. Guinta re: his ability to add length and girth to some patients with implants. I am just wondering if the length he is proposing is attainable if you get a lig-cut with him, or if the actual stem cell matrix does some lengthening? This stem cell stuff is really interesting to me, but I still cant figure out how Dr. Runels and Dr. Guinta can add legitimate length to the penis by injection? Dr. Runels isn't sharing (yet?), and while I haven't spoke with Dr. Guinta, he doesn't offer any technical details on his website. I tried submitting posts 3 times today-lets see if this one goes through. I think its fitting to reply to this thread seeing that I'm sitting on a train returning home from my 2nd staged of surgery (1 year anniversary of the first procedure). All went very well-minimal discomfort. Every case I'd different, mine included lipo of a substantial suprapubic fat pad and excision of some redundant skin (I used to be heavy but that seems to be a stubborn area to lose fat). Pe is real and my journey will consist of probably 1 more top up, more stretching (NO PUMPING FOR A YEAR DUE TO THE GRAFT) and gather more info on the matrix shot ...might do that to. But this is where my pe journey will end. I believe pe surgery is potentially dangerous and should only be undergone for those who really need it. All are not created equal and the best candidates are those who suffer emotionally from micro or. From being marginally "average". I don't have much patience for guys w/8 inch penises complaining that they're not 10. My blogging days will be behind me soon, as I pan to move on- this is "penis wkend" so to speak....having my surgey (done) and moving on b/c I must. My advice to newbies (advice I wish were given to me): stretch/pump intelligently for @ least a year. If your a candidate-go fft route and continue to augment until your hearts content. (No pumping for min. Of 6 mos. After surgery or you'll lose the graft). Keep your finger on the pulse of the matrix shot. Don't put anything foreign into your body-that includes plastic in all forms. The doctors I trust most on this scene are: Gary Alter,MD and Stephen Giunta,MD I'd love to know more about Runnels but need to learn much more. For the stat lovers among you: Pre-op Length-"0"- Beat that! Due to fat pad and redundant skin Bone pressed-2 inches Erect-5" on the button Post op (first op included lipo/skin removal lig release and fft) 3 inches flaccid (Gained another 3/4 of an inch stretching for 1 year Erect-now a hair under 7" My girth was never too bad probably between 4.5-5" Pre-op (just b4 yesterday 5.5....not sure what it'll be after this round. My goal is to solidify the 7". Length marker by continuing the stretching a bit more and continue to work on the girth til I acheieve my desired 7X7 goal. hi guys- Sheldon thank you for bringing this post to my attention, I didn't know I was being asked a question! Things are going well for me, as I anticipated I will most likely require another treatment to be where I want to be girth wise. In recent months I also slacked on the stretching. I'm sure @ this point I'm not meant to be over 7" in length for sure and that's fine by me as long as I solidify that marker. Still want to know more about prfm!
As far as your questions via pm, I'll answer here. No coke bottle, corn dog effect. To me it looks and feels natural. In my opinion the guy in the photo you sent via pm from Giunta's site has an excellent result. I need another treatment to acquire a heavier appearance like that but I think its worth it.
As far as the photos of the deformed penis (w/ lumps of fat), these results are obviously poor. Not sure who did that, but these are not typical results from someone who knows what they're doing! The lump on the top of the penis actually looks like a bad dermal or fat strip, which contracted. Immediately post op the penis may look hard and slightly lumpy no doubt but this is a temporary condition, it softens and becomes very natural after the healing period of 6-8 weeks. Its important not to panic during this time! If someone take a photo of any immediate post op pe procedure chances are, its not going to look great. But I digress, not sure who the man in the link above went to but it is a poor result for sure as a result of poor technique...certainly doesn't look like Giuntas work. I didn't notice the lengthening incision if its a VY it was probably done out of The States, this is indicative of much less sophisticated techniques. Finally, I'm postive that the reason for displaying these photos is generate business for a doctor who performs a different procedure...Beware my friends- lots of dangerous people out there! And remember "triple augmentations", meaning pe surgeries which include surgical implantation of material to the coronal ridge of the glans has resulted in serious, serious problems such as complete inability to achieve/maintain erection-way to many nerves to tamper w/this extremely delicate area.

PRP is used in other forms of fat transfer and the results, from what I've read, are often disappointing. There is a plastic surgeon in NY, who does a face lift style prodcedure, using PRP fat transfer. All the articles about it are very positive, but the actual patient reviews were pretty negative. I'll try and remember his name, so I can link the site. Personally i'd be skeptical of any advances in FFT. Centrefuge and insulin were touted as the answer years ago, with lots of published reports providing evidence supporting the theory. But then came other studies that showed it made no difference and many surgeons claim the same. If there really was an effective way of ensuring fat graft survival, then the ultimate soft tissue filler would have been created and the likes of Restylane, Juverderm and Radiesse etc, would no longer be used.
For the last decade Giunta has often updated his FFT technique and made a big deal about the improvement on his site, but from the odd reports we see on messageboards, satisfaction rates are about the same as they have always been. According to Dr Reed, 15 years ago, when he was doing FFT, about 70% of his patients were pleased with the results. Id' say the same is still true today, even with Giunta's improvements. On MNS there were about 4 guys who were pleased with recent FFT with Giunta and 2 (Dan and the guy who had necrosis) that weren't. On thundersplace there was a guy who recently posted about FFT with Giunta and he is now pleased with his results, but said it took 6 top ups to get things right. This supports my theory that the biggest improvement in FFT is that surgeons and patients now realise that overfill isn't the way to go and one must accept that to get a good result the chances are you will have to return at least once to get things smoothed out. I'm not saying PRP doesn't have it'd advantages and I'm sure that there are plenty of published studies which will say it does. But from my experience of following PE for years, theory and published papers are pretty worthless compared to hearing about actual patient results. And from what we've read here and on other messageboards about FFT in the last year or so, I'd say results are similar to what they've always been. If a guy needs to go back for 6 top ups, to get things evened out, then it doesn't say a great deal for PRP. I must say though, that FFT results, again based on the small sample of messageboard patients, seem to have a far higher success rate than many sources want you to believe. I know of quite a few people now who are very happy with their FFT results, such as IDM, which is contrary to so much of what one reads on sites touting other methods.

According the Dr. Giunta's website: drgiunta.com/fft-platelet-rich-plasma.html:'The commonly accepted method of harvesting/isolating Platelet Rich Plasma is by using centrifuge separation of the patient’s blood into the three levels defined by spin separation. Once the enriched and concentrated PRP is collected, Dr. Giunta introduces it into the harvested adipose tissue, then leaves it to incubate for a period of time to release growth factors and other compounds into the mix in the appropriate amounts. Finally, it is reintroduced in the penile shaft where it will aid in the fat retention and healing processes. nategonewildAs for the angle of the penis changing, I've heard various accounts that cutting the ligaments do or don't effect the angle, hopefully someone with the procedure can chime in. It is possible to get all three done, but in my personal opinion, length is rarely effective and glanular is on the high risk side. Shaft girth enhancement is also risky but by far the most effective when seeking results from any number of procedures. As for length, I believe looking into stretching regimens may yield similar or better results than just ligament cutting (besides, you have to stretch rigorously post-op after a ligament procedure anyways). Glanular enhancement can be done conceivably, but you are going to want to look into temporary fillers since this is a part of the penis anatomy that isn't so easy to enlarge permanently. As for overall (shaft) girth, there are many options but all with varying degrees of risks: Alloderm/Belladerm, Free Fat Transfer, Dermal Fat Grafts, Prosthesis Implant, and PMMA. Silicone gel is considered ineffective and dangerous, and that is one method that PhalloBoards can declare as a poor method. As for FFT, there is a more improved approach with this procedure using Platelet Rich Plasma which is supposed to allow the fat to survive better after injection. They also stage the injections so that overfilling doesn't suffocate the fat of much needed blood supply, etc. Our Moderator IDM can be Private Messaged, he had the FFT-PRP done with Dr. Giunta.

hi guys I had fft just over a week ago. Still swollen and part of penis taped up from scar revisions so too early to say if things will be ok. How long till the pain in night time erections goes away? There a killer, about every hour I wake up due to the pain. Makes for a terrible sleep!
Have a follow up appt in a couple of days , but pretty sure the doc told me he injected 30ml.

hi skeptical 1 Dr Caminer , plastic surgeon in sydney before surgery girth was 4'' flaccid - 4.5'' erect , post op 6'' flaccid but there is some swelling and I know some will go.
Also had scarring from previous botched surgery from Dr Djinovic corrected.
To early to make a call on the sucess of the FFT but if I can talk anybody out of going ahead with either of the Scaffold drs In Europe I will. I have met both and since surgery with one of them have spoken to many people that had botched surgeries.
