Some thoughts:
First, and w/all do respect to my fellow pe brethren and to our friends throughout the world, the fact that men would consider going to places like Russia, Serbia and Brazil to augment their penises by physicians who we can't communicate with (really) and whose only credentials consist, for all intents and purposes, of a photo or two is disconcerting.
One must be very skeptical these days! The gentleman in the photo has a decent looking peniis yes. Assuming he actually had pe surgery, I'm curious what he looked like pre op. His flaccid length is average or better, and I personally never saw photographic evidence (again assuming it can be taken at face value) of Post op results which yield significant length gain w/the exception of certain cases of men who've received the silicone prosthetic...most of you know my sentiments on that!
I guess my point and friendly advice is let's not let our zeal for obtaining a larger phallus cloud our judgement!
I get the sense that if there were reports of witch doctors performing rites of penile enlargement in remote 3rd world countries, some would consider it, if the photos were decent- Let's keep our wits about us!
I've traveled extensively throughout the world...on 2 occasions I got sick while abroad. I can say whole heartedly that the best medical care in the world (IN MY OPINION) is in The U.S.
No disrespect intended to anyone.

AND BTW,
From a scientific point of view, muscle is more dense than skin and may hold up "longer" than a dermal graft but it is subject to degeneration and failure over time. The reason these graphs fail is b/c they require greater blood supply for them to maintain their integrity.
It seems adipose (fatty tissue) has an easier time developing a blood supply. Though reports are showing that fat retention rates are higher in fat grafts than in the past due to harvesting techniques and to the treatment(s) of the tissue, the potential downside for some maybe the fact that fat is less dense than muscle tissue.
This may only be considered a negative, if one is seeking a significant augmentation (more than an inch in girth and if the patients erection quality not optimal.
