I believe a member has posted this idea here before and after additional research I am inclined to believe that the primary cause for length loss is rooted in the skin of the penis having to accommodate the additional volume caused by the implant by having to sacrifice length initially.
The skin has a limit in terms of pliability and if that limit is surpassed than something has to give and usually that is length. This could be negated by getting an implant that isn't too big initially however if one wanted to eventually end up with a very large girth gain than one would have to go back and have another procedure in which a large implant would be implanted once the skin had stretched and become accustomed to the initial implant.
This comes with the risk of infection via each post op healing experience.
Skin necrosis can also be caused by too much pressure on the skin at which point it finally gives and the immune system "attacks"/rejects the implant in an effort to rid itself of the cause of the problem.
If you lose length initially, then the scar tissue that builds over the ensuing time frame after the operation heals itself to the length that is attained post op.
I do feel that the scar tissue is malleable hence the success of some men regaining length with hanging post op although it requires a lot of work.
I do think after seeing pictures of guys who injected themselves with liquid silicone over a long period of time that it is possible to stretch the penis skin to incredible size, but the key here is that its done over time.
The only thing I can think of to help initiate the possibility of having a fairly large implant put in through the first operation would be to undergo a pumping regiment to help stretch the skin of the penis. I would assume that a lubricant/lotion that would help skin healing/care would be important as well.
Some guys are genetically lucky and have very malleable skin. Or their flaccid to erect volume ratio is quite large implying that their penile skin is already quite malleable allowing for a larger implant while not losing length.
